Bright Eyes Ahead: Understanding the Difference Between Under-Eye Filler, PDGF & PRP Boston Medical Aesthetics

Tired eyes, dark circles, and hollowness under the eyes are common concerns we hear at Boston Medical Aesthetics. The good news is, there are multiple treatment options to help you look more refreshed and well-rested. Three of the most popular approaches include under-eye filler, PDGF (platelet-derived growth factor), and PRP (platelet-rich plasma).

While they can all improve the appearance of the under-eye area, each treatment works differently and comes with its own benefits and risks. Here’s what you need to know:

Under-Eye Filler

What it is:

Restylane Eyelight, a hyaluronic acid-based injectable, is used to fill volume loss in the tear trough area. Our under-eye filler treatment replenishes structure while remaining flexible enough for natural movement, providing targeted tear-trough support.

How it works:

Filler physically restores volume and smooths the transition between the lower eyelid and cheek, minimizing the appearance of hollows or deep circles.

Benefits:

  • Immediate results
  • Smooths out shadowing and hollowing
  • Long-lasting (typically 9–12 months)
  • Minimally invasive with little to no downtime

Risks:

  • Swelling or bruising
  • Tyndall effect (a bluish tint under the skin if filler is placed too superficially)
  • Risk of vascular occlusion (rare, but serious—why choosing an experienced injector is essential)

PDGF (Platelet-Derived Growth Factor) Injections

What it is:

A lab-created growth factor that stimulates tissue repair and regeneration. Unlike PRP, there’s no blood draw, and PDGF+ is formulated for consistent, concentrated results. This PDGF growth factor therapy is available in our practice and offers a precise alternative to PRP, delivering collagen-stimulating under-eye care.

How it works:

PDGF helps rejuvenate the under-eye area by stimulating collagen and elastin production, improving skin quality, tone, and texture over time.

Benefits:

  • No blood draw required
  • Clinically backed formulation with precise dosing
  • Can improve crepey skin, dark circles, and mild volume loss
  • No downtime

Risks:

  • Mild swelling or redness at the injection site
  • Results take time and may require a series of treatments
  • Not a replacement for volume loss in deeper hollows

PRP (Platelet-Rich Plasma)

What it is:

A treatment using the patient’s own blood. After drawing and spinning the blood in a centrifuge, the plasma rich in growth factors is injected under the eyes. PRP under-eye rejuvenation relies on your own biology to stimulate repair.

How it works:

PRP stimulates the body’s natural healing process to improve skin texture, tone, and firmness.

Benefits:

  • Natural, since it uses your own blood
  • Improves fine lines and pigmentation
  • No risk of allergic reaction

Risks:

  • Requires a blood draw
  • Results can be inconsistent and depend on your overall health
  • Multiple treatments often needed for best results
  • Mild swelling or bruising

Which Treatment Is Right for You?

That depends on your specific concerns. If you're dealing with deep hollows or want instant results, under-eye filler may be the best option. If your main concern is texture, skin thinning, or crepiness, PDGF injections could be ideal. And if you prefer an all-natural approach, PRP may be worth considering (although we do not currently offer PRP at Boston Medical Aesthetics).
